"Stuart provides on-site nutritional risk screening, nutritional assessment and counseling, bioelectrical impedance analysis, nutrition classes, and nutrition education.
 
Number of Clients and Specific Population Served: The JGC provides comprehensive primary care services to adults aged 18 and older, particularly medically uninsured and underinsured, who are living with HIV infection. JGC targets persons living in Los Angeles County with a particular emphasis on the gay, lesbian, bisexual and transgender community. JGC is located within one of the epicenters of the HIV epidemic in Los Angeles (the Hollywood/Wilshire Health District) and the clinic’s patient population reflects both the diversity of Los Angeles and the demographics of HIV infection in Los Angeles County. JGC currently serves approximately 1,800 patients per year: 96% Male of which 90% identify as men who have sex with men; 2% Female all of whom identify as women who have sex with men; 2% Male to Female Transgender; 43% White; 40% Hispanic; 12% African-American; 5% Asian/Pacific Islander.

Are all patients screened for nutritional risk? All new patients are informed of Stuart’s presence in the clinic as part of the intake process. They are strongly encouraged to see him, especially if they have nutrition concerns or they are at increased nutritional risk. Established patients are strongly encouraged by medical providers to see Stuart when the risk of poor nutritional status is increased. Stuart has designed referral forms that medical providers complete and pass on to him. He notes, “I use these forms to call people. If they do not have a scheduled appointment, I let them know that their medical provider would like them to see me. These forms also inform me of what I should be ‘looking for’ when I see the patient.” We are implementing procedures requiring patients to see the Registered Dietitian as part of the initial intake process.

How often do you provide nutritional assessments and follow-ups? This is done per Los Angeles County requirements. Patients are required to see the Registered Dietitian at least twice a year – more depending on co-morbidities and what their level of care is. Assessments are done at the time of the initial visit, if there is significant change in health or medical history,  as well as annually.
