Medication Interactions
As prescription drugs become more commonly used in our
society it is vital that we stay current on the interaction of prescribed
drugs and other substances. As noted in earlier editions of the HIV ReSource
Review and HIV Nutrition Update, HIV-positive individuals have numerous
reasons to avoid alcohol, certain herbs, illegal substances, and other
compounds to avoid interactions with prescribed medications. As noted in
Issue 27 of the HIV ReSource Review, certain herbs have the potential to
increase blood glucose with the quantity ingested relevant to the extent
of the interaction experienced by the user. This is especially important
to know for people who have diabetes or lipodystrophy, which often includes
glucose abnormalities. St. John's Wort (Hypericum perforatum) affects drug
levels of many medications including commonly used
statins to lower cholesterol and antiretrovirals used
for HIV infection.
Medications increasing the risk of glucose abnormalities
include drugs such as beta-blockers, corticosteroids, glucocorticoids,
human growth hormone, megace, nicotinic acid, pentamidine, products with
estrogen, protease inhibitors, steroids, and thiazides.

Congress Blindfolds Consumers- Removing the Right to
Know What is in Your Food
On March 8, despite massive public opposition, including
50,000 calls and letters from supporters of the Organic Consumers Association
(OCA), the House of Representatives passed the controversial "national
food uniformity" labeling law, which would eliminate over 200 state food
safety labeling laws. The law basically takes away local government and
states' power to require food safety labels such as those required in California
and other states on foods or beverages that are likely to cause cancer,
birth defects, allergic reactions, or mercury poisoning.
The bill would also prevent local municipalities and states
from passing laws requiring that genetically engineered foods and ingredients
be labeled. Under the bill, hundreds of state laws and regulations would
be eliminated, including those relating to the safety of milk, fish, and
shellfish. In order to become law, the bill will now have to go to the
Senate for a vote. Because of the enormous public backlash against the
bill, Washington analysts believe the bill will have great difficulty passing
in the Senate. OCA and other public interest organizations have vowed to
go "all out" to stop this anti-democratic, anti-consumer bill in the Senate.

FDA Issues Draft Guidance For The Safe Production Of

To minimize microbial food safety hazards common to the
processing of most fresh-cut fruits and vegetables sold to consumers in
a ready-to-eat form, The Food and Drug Administration (FDA) today published
a draft guidance document for producers of fresh-cut produce entitled "Guide
to Minimize Microbial Food Safety Hazards of Fresh-cut Fruits and Vegetables."
"Fresh cut produce is the fastest growing sector of the
fresh produce industry. This document should help to improve safety
by providing clearer guidance on how to reduce health hazards that are
potentially introduced during the production process," said Acting FDA
Commissioner Dr. Andrew von Eschenbach.
Processing produce into fresh-cut produce increases the
risk of bacterial contamination and growth by breaking the natural exterior
barrier of the produce by peeling, slicing, coring, trimming, or mashing
with or without washing or other treatment before being packaged for consumption.
Examples of fresh-cut products are shredded lettuce, sliced tomatoes, salad
mixes (raw vegetable salads), peeled baby carrots, broccoli florets, cauliflower
florets, cut celery stalks, shredded cabbage, cut melons, sliced pineapple,
and sectioned grapefruit.
This draft guidance discusses the production and harvesting
of fresh produce and provides recommendations for fresh-cut processing
in several areas--
(1) personnel health and hygiene, (2) training, (3) building
and equipment,
(4) sanitation operations, and (5) fresh-cut produce
production and processing controls from product specification to packaging,
storage and transport. The final chapters provide recommendations
on recordkeeping and on recalls and tracebacks. The guide complements
FDA's Current Good Manufacturing Practices regulations by providing specific
guidance on the processing of fresh-cut produce.
In the draft guidance, FDA recommends that processors
encourage the adoption of safe practices by their partners throughout the
supply chain, including produce growers, packers, distributors, transporters,
importers, exporters, retailers, food service operators, and consumers,
to ensure that the processor's efforts will be enhanced. These practices
include:
·
Establishing a company policy that employees report any active case of
illness to supervisors before beginning work and training;
·
Training supervisors to recognize typical signs/symptoms of infectious
disease; maintain the proper first aid to protect and cover any wound;
and not allow an employee to work with any aspect of fresh or fresh-cut
produce, processing equipment or tools until the wound has healed and/or
the infectious disease has been treated.
The guidance also recommends that fresh-cut processors
consider a preventive control program such as the Hazard Analysis and Critical
Control Points (HACCP) system to build safety into the processing operations
for fresh-cut fruits and vegetables. HACCP is a prevention-based
food safety system designed to prevent, eliminate, or reduce to acceptable
levels the microbial, chemical, and physical hazards associated with food
production.
FDA believes awareness of the common risk factors discussed
in this guidance, and implementation of preventive controls determined
by a firm to be appropriate to its individual operations, will enhance
the safety of fresh-cut fruits and vegetables.
Consumers can reduce their risk of illness from fresh-cut
produce by following safe handling practices such as refrigerating the
product after purchase; using only clean hands, utensils or dishes in preparing
the product; and discarding the product when the "use by" date has expired.

Dietetics Practice Group (DPG) of the American Dietetic Association (ADA)
co-sponsored the "5th Annual NIAC & HIV/AIDS DPG Conference: Putting

"Vitamins B, C and E Beneficial for Infants Born to HIV-1-Positive
Mothers"
NEW YORK (Reuters Health) Feb 14 - Maternal multivitamin
supplements containing vitamins B, C and E reduce the risks of motor developmental
delays in infants born to HIV-positive mothers in developing countries,
according to a new study in Tanzania, Africa.
These findings support current recommendations to give
multivitamin supplements, in addition to an anti-retroviral treatment,
to HIV-1-infected pregnant women in developing countries, note the authors
of the study, which appears in the February 2006 issue of Pediatrics (Pediatrics
2006;117:216-25.). Supplement administration has already been associated
with delays in HIV progression and reduced complications such as low birth
weight, preterm birth, and fetal death.
The team of researchers, led by Dr. Nuala McGrath from
the Harvard School of Public Health in Boston, set out to determine the
relation between maternal multivitamin supplementation and the mental and
psychomotor development of children born to HIV-1-positive mothers.
A population of HIV-1-infected women received daily doses
of either vitamin A alone, a multivitamin regimen of vitamins B1, B2, B6,
niacin, B12, C, E and folic acid, both supplements combined, or placebo.
The treatment lasted throughout pregnancy and continued for 18 months after
delivery.
The researchers used the Bayley Scales of Infant Development
to assess overall cognitive and motor scores at 6, 12 and 18 months in
327 newborns. They measured the Mental Development Index (MDI) and the
Psychomotor Development Index (PDI).
Their results indicate that a multivitamin supplement
(B, C and E without A) had significant effects on motor scores, increasing
PDI scores by 2.6 points.
"An increase of 2.6 points in the average motor index
of a population results in a 35% reduction of the number of individuals
who have an index score < 70 and would require greater educational resources,
medical care, and other social supports," the authors write.
These effects were variable across subgroups and suggest
that children born to women with a proxy for advanced disease - a high
erythrocyte sedimentation rate - are those who benefit the most from multivitamin
supplements.
The team also found that the multivitamin regimen was
significantly protective against motor developmental delays (relative risk
0.4), but did not affect mental scores significantly.
Vitamin A, on the other hand, was not associated with
a change in either mental or psychomotor function, nor did its combination
with the multivitamin supplement improve DPI or MPI scores.

Editors Note: Members of AOL and other subscribers
may not be getting their updates due to web hosting spam blockers. Unfortunately,
many people who send unwanted email, use the Internet to 'mine' and use
other people's web site domain addresses to send their 'junk mail' (spam).
As many, many, others, our main web site domain address has been targeted
to send unwanted mail. Most 'junk mail' is sent from illegal addresses
(such as xyqpsuzy@, etc.). Members of this list receive our usual weekly
email on Friday and it comes to you with the sender listed as this list
name. People sending junk mail include our main domain name but usually
do not use any of our valid email addresses. We do not send attachments
