Dietary Fish Oil Supplementation

As noted in the November 25, 2005 CATIE News, fish oil can safely lower lipid levels. HIV ReSources first reported on the potential of dietary fish oil supplementation in the article “Have Fat Will Travel” in Issue 16 of the HIV ReSource Review. Other articles in the HIV ReSource Review including: “Heart Healthy Food Choices in the Era of HAART: Part I”, “Heart Healthy Food Choices in the Era of HAART: Part II”, and “Blood Glucose Abnormalities In HIV-Positive People” also discussed dietary fatty acid supplementation. 

Numerous issues of the HIV Nutrition Update educated readers of ongoing and finished studies involving the use of omega-3 fatty acids. “Omega-3 Fatty Acids And The Metabolic Complications Associated With HIV Lipodystrophy” and "Cardiovascular Disease Risk in HIV Infection: Rationale for Diet, Exercise, and Healthy Lifestyle” discussed information on a diet intervention study at the Tufts University School of Medicine. Numerous other potential benefits from the dietary intake of omega-3 fatty acids were noted in the Update Central column of Issue 44. An important point for people with bone disorders was noted by Diana Johansen, RD in the article “Bone Loss in HIV-Positive Women: A Canadian Study”. Diana noted that omega 3 fatty acids may help to build bone protein, which helps lay the foundation for minerals. 

Daily omega-3 intake recommendations for healthy people are based mainly on age and range from between 0.5g/day for infants to 1.6g/day of alpha-linolenic acid for men. People wanting to obtain omega-3 fatty acids from food can read information about calculating daily fat goals, ratios of saturated, monounsaturated and omega-3 and omega-6 fatty acids in a book titled The Healthiest Diet in the World

Although pilot studies continue to note that dietary fish oil supplements may be beneficial for people with HIV/AIDS, it is important to note: 

* an increase in total cholesterol may occur with supplements; 
* people with low platelet levels increase their risk of spontaneous bleeding; 
* mild side effects such as nausea, constipation and diarrhea can occur; and 
* consuming fish high in omega 3 fatty acids (such as tuna or salmon) can reduce plasma triglycerides without affecting low-density or high-density lipoprotein cholesterol
levels, however the mercury content of certain fish must be considered.

HIV Nutrition Update - Newszine Winter 2005 Issue

The Winter 2005 HIV Nutrition Update Newszine issue is freely available to the public in Adobe PDF format through the Internet. Subscribers can view all HIV Nutrition Update issues in PDF and HTML format at by using their assigned username and password.

This issue highlights the usefulness of Traditional Chinese Medicine (TCM) methods Gua Sha and Ba Guan. Richard Allen Freiberg’s article offers information on the use of soft tissue manipulation methods that combine Gua Sha and Ba Guan into a treatment known as Ba Gua Fa. This effective TCM method helps people in pain, particularly those with disabling health conditions that are not helped by Western medicine. Like the knowledge in Richard’s previous HIV Nutrition Update articles (Issues 32, 35, 37, 40, 42, 44- Previews), this valuable information offers a view of how TCM helps people with various illnesses.

Sharon Ann Meyer’s article highlights the need for awareness of how vital it is to determine energy needs, especially for people with medical conditions such as hepatitis. As the use of highly-active antiretroviral therapy (HAART) continues, resulting in metabolic abnormalities, knowing about body energy needs is more important than ever before.

Information on Non-Nucleoside Reverse Transcriptase Inhibitors helps readers to decide if taking these antiviral agents may be helpful. Antiretrovirals And St John's Wort 

St. John's Wort (Hypericum perforatum) affects drug levels of many medications including commonly used statins to lower cholesterol and antiretrovirals used for HIV infection. Although a recent test tube study suggests that St John's Wort protein may suppress HIV, it is important to note that this is only one study completed in a laboratory. It is always prudent to investigate both the author and publication of
seemingly useful information. For people taking medications it may be wise to avoid the use of St. John's Wort to limit possible herb-drug interactions. 

Previous issues of the HIV ReSource Review ( Issue 21  and  Issue 22 ) note that St John's Wort can interact with antiretroviral treatment. Numerous precaution sections within antiretroviral product inserts warn users about drug interactions for people using St. John’s Wort. The concentrations of some antiretrovirals such as efavirenz (Sustiva) can be increased. St. John's Wort however, may lower blood levels of protease inhibitors such as indinavir (Crixivan) and ritonavir (Norvir). Taking the herb along with some protease inhibitors can stop the medications from working. This can cause HIV to become resistant to antiretrovirals, which may lead to depressed levels of immune system cells and more infections. 

As noted in issues of the HIV Nutrition Update ( Issue 37  and  Issue 52 ), herbal supplements, including St. John’s Wort, can be contaminated with a variety of microorganisms. Physicians treating people with HIV need to be aware of supplement use to avoid adverse effects that could arise between their use along with conventional treatments and medications. Objective information on the use of herbal and other
supplements along with discussion of the harm: benefit ratio can keep people informed about supplement-drug interactions.

Food Security and Food Safety

As previously noted, food insecurity is highly prevalent in the United States and continues to increase due to hurricanes, with the latest being Wilma. Positive people can get the nutrition care and support they need through community kitchens such as those listed at the Association of Nutrition Services Agencies (ANSA) web site. Numerous materials and resources are available through ANSA. Also, visit the Food And Recipe Resources web page

Protecting the food supply is more important than ever before. The U.S. Food and Drug Administration (FDA) has  guidance documents designed to help manufacturers minimize the risk of tampering or other malicious, criminal or terrorist actions at http://www.fda.gov. We believe that the incidence and severity of these storms will continue to increase so it is vital to learn what you can do to stay safe. Since the passing of my 37-year old son due to a motorcycle accident on 4/4/04, I have sensed that my mission is to now broaden my efforts toward a more comprehensive goal serving humanity in a different way. Bird Flu Alert 

As people with compromised immune systems are more susceptible to viruses of every type it may be prudent to learn more about bird flu, which is becoming more common among chickens and ducks. The H5N1 strain of bird flu was first identified in 1997. It is an especially virulent strain that has spread from Asia to Europe; it killed 60 people in Asia. The H5N1 strain of bird flu was found in Turkish poultry and may also be in Romanian poultry. Bird flu has also been detected in Hong Kong in 1997 and in the Netherlands. Wild birds may be carrying the H5N1 virus along major migratory pathways. 

Antivirus drugs to treat the bird flu must be taken within 36 hours of contamination to be most effective. Tests on wild birds and poultry are now being done in Britain to find out the potential threat of an avian flu epidemic. Vulnerable groups such as some people with compromised immune systems, the elderly and children with asthma or diabetes should be vaccinated against the three regular flu strains. Seasonal flu vaccinations are also important because they could reduce the chances of the bird flu virus mutating into a form that moves more freely between people.

Update on Plant Stanol Esters And Cholesterol

The nutritional aspects of lipid and lipoprotein abnormalities have been discussed in the HIV ReSource Review and various weekly messages from this electronic list. Research continues to show that margarine-like spreads of free soyabean-oil sterols are effective in lowering both total and low-density lipoprotein (LDL) cholesterol levels. Commercially more available than in previous years, margarine spreads with plant sterols/stanols are also less costly than when research first began to show how useful they were for people with fat and lipoprotein abnormalities. Low saturated fat, low cholesterol meal plans with increased fiber and use of these margarine spreads containing plant sterols/stanols (1.5-2 grams per
day- found in about five teaspoons of margarine) can be beneficial to people with these conditions. 

Recently published articles note that dietetics professionals treating people with high cholesterol should now feel comfortable suggesting the use of these margarine spreads along with offering dietetic counseling for optimal effectiveness. (1-2) Another recently published article notes that policosanol (A natural supplement derived from sugar cane.) is also well tolerated, safe and more effective than plant sterols and stanols for favorably altering the lipid profile and reducing LDL level. (3) Although the authors note that policosanol seems more effective at approaching antilipemic drug efficacy, since it is derived from sugar cane it may not be the best choice for people with uncontrolled glucose abnormalities.

New nutritional challenges emerge for HIV care providers

Nutritional therapy for HIV-infected patients is shifting focus. Drug treatments designed to combat the HIV virus have improved, decreasing some nutritional problems, while bringing others to light. As researchers from the Nutrition/Infection Unit in the Department of Public Health and Family Medicine at Tufts University School of Medicine describe in an editorial review and other research reports, new nutritional challenges in HIV/AIDS care have emerged. 

The changes, according to Alice Tang, MS, PhD, assistant professor at Tufts University School of  Medicine, and her colleagues, have resulted from the introduction of an advanced class of drugs called highly active antiretroviral therapy (HAART) over a decade ago. 

In the past, the nutritional component of HIV/AIDS care was directed toward two major goals: supplying the vital nutrients that were often lost through poorly-functioning digestive tracts and minimizing weight loss. "The newer medications keep patients healthier overall, so we are not dealing with quite as much severe weight loss," says Tang. "Now we are able to concentrate more on determining the vitamin and mineral ('micronutrient') requirements of these patients and investigating potential therapeutic uses of supplements." 

This is particularly important in the age of HAART, according to Tang, who has researched the role of antioxidants, like vitamin E, in HIV extensively. As it turns out, HAART medications, while controlling disease progression for many patients, may also increase oxidative stress. "Oxidative stress" refers to the state of imbalance that exists when chemically unstable by-products of metabolism exceed the body's capacity to quench them, either with its own natural antioxidants or with those obtained from the diet. The potential result of this imbalance is damage to cells and tissues. "Research efforts must focus on determining if this is, indeed, true. If so, what is the mechanism? Will dietary or supplemental antioxidants help?" asks Tang. 

Another area of active research underway at Tufts and elsewhere relates to what Tang calls, "one of the most vexing issues facing HIV-positive individuals." In fat redistribution or lipodystrophy syndrome, "body fat changes may include fat loss under the skin (on the trunk, extremities, buttocks or face) and/or fat gain (in the abdomen, back of the neck or in the breasts)" explains Denise Jacobson, PhD, MPH, assistant professor at Tufts School of Medicine. Patients with this condition may also have abnormal metabolism of fat and/or sugar. Jacobson says, "we are still learning about these changes, some of which may be related to HAART." 

Over the years, the Nutrition/Infection Unit, led by Sherwood Gorbach, MD, professor of medicine, and Christine Wanke, MD, professor of medicine, has been active in nutrition and HIV research. In the current paper, which is just one of Tang's several reviews analyzing the research in the field, she summarizes the findings of studies looking at the roles of vitamins B12, A, and E in HIV transmission and disease progression. Results are mixed and inconclusive, but Tang says that these findings help to guide researchers in future endeavors. She says, "it is clear that micronutrients play a critical role in the maintenance of immune function and overall metabolism. We need to concentrate on defining the areas in which they may help to maximize the health status of HIV-infected patients."

Computer Email Warning

HIV ReSources noticed several emails in the past week supposedly coming from our own server that we did not send. Excluding the emergency email message about hurricane Katrina, please be aware that HIV ReSources sends your weekly email updates only on Fridays. Sometimes members will unintentionally send these unwanted messages with advertisements or viruses when their computer gets infected by these email scams. When a computer is infected, it sends messages to people listed in their address book.  This can also occur by advertising agencies that scan the Internet for valid email addresses. Sometimes, the message has a file attachment that contains a computer virus. When the message attachment is opened, it infects the users’ computer. 

HIV ReSources never sends attachments or advertisements except those for our own products. As many of you, HIV ReSources deletes numerous copies of so-called spam (junk/unsolicited) email laden with viruses. Please keep your anti-virus software up-to-date to avoid infection. And never open an email attachment unless you are expecting it and know it is uninfected.

Considering the increased susceptibility of HIV-positive people to foodborne illness, it's prudent to stay up-to-date on food security and food safety issues. Food insecurity, highly prevalent in the United States, is now increased by Hurricane Katrina. Positive people can get the nutrition care and support they need through community kitchens such as those listed at the Association of Nutrition Services Agencies (ANSA) web site. Protecting the food supply is more important than ever before. The U.S. Food and Drug Administration (FDA) has  guidance documents designed to help manufacturers minimize the risk of tampering or other malicious, criminal or terrorist actions.
