FDA Public Health Advisory for Nevirapine (Viramune)
Recent safety-related changes have been made to the nevirapine
(Viramune) label (package insert) including information on the appropriate
use of HIV triple combination therapy containing nevirapine. The chance
of liver toxicity is associated with long term nevirapine use. Both clinically
symptomatic and asymptomatic liver toxicity are observed with long term
use of nevirapine in combination with other HIV drugs. Females and patients
with higher CD4+ cell counts are at increased risk of liver toxicity. The
Indications and Usage section of the Viramune label now recommends against
starting nevirapine treatment in women with CD4+cell counts greater than
250 cells/mm3 unless benefits clearly outweigh risks. In addition, the
revised label now includes a Medication Guide to inform patients about
risks associated with nevirapine when used for the treatment of HIV. For

The Food and Drug Administration held its Antiviral Drugs
Advisory Committee meeting on May 19, 2005 at the Hilton Washington DC
North Hotel in Gaithersburg, Maryland. The committee discussed drug application
(NDA) 21-814, proposed trade name APTIVUS (Tipranavir) 250-milligram capsules,
Boehringer Ingelheim Pharmaceuticals, Inc., indicated for the treatment
of patients with HIV.

research results related to nutrition and HIV- positive people in the International
HIV Nutrition Update newszine. Articles that focus on the successes and
problems of attempting to conduct research related to nutrition and HIV
are helpful to clinicians considering such research.

Dietary Supplement Update
Numerous issues of the HIV
ReSource Review and the HIV
Nutrition Update have detailed the use of dietary supplements among
people with HIV. The Summer 2005 HIV Nutrition Update includes articles
on the interaction of herbs and HIV medications and herbs that affect blood
sugar levels. As noted in previous newsletter issues, research investigated
the effect of garlic intake. Garlic is one of the top 10 complementary
substances used by those living with HIV. Studies show that garlic
can inhibit nuclear factor- KappaB (NF-kB) a cellular transcription
factor that is important in the control of inflammatory responses.
Garlic may have immunomodulatory and anti-inflammatory
properties that result in decreased NF-kB activity in human blood cells.
In the Sept/Oct
2001 HIV Nutrition Update we noted that it might be wise for people
who use saquinavir to avoid the use of garlic supplements but cooked garlic
may be okay to eat.

Food Safety
As noted at the Centers
for Disease Control and Prevention web site, “many people don’t think
about food safety until a food- related illness affects them or a family
member.” When individuals already have illness that causes symptoms similar
to those of foodborne illness it can be difficult to recognize that those
symptoms may be due to food or water-borne illness.
CDC estimates that 76 million people get sick and 5,000
Americans die each year from foodborne illness. Preventing foodborne illness
remains a major public health challenge. We all eat and many
of our subscribers donate food to food banks, work at food banks, or get
foodstuffs from food banks so it is vital to know about food safety and
food recalls. These recent food recalls noted by the Food and Drug Administration
(FDA) may be of importance to subscribers.
* FDA recently announced that SeaSpecialties of Miami,
Florida put out a recall for Mama’s Smoked Salmon Nova Snacks due to contamination
with Listeria monocytogenes. The products are in four and 8-ounce packages
distributed on the east coast of the U.S. Both products have a ‘sell by’
date of August 10, 2005 printed on the front of the packages. Listeria
monocytogenes is an organism that can be serious and sometimes cause fatal
infections in young children, frail or elderly people, and others with
weakened immune systems such as those with HIV. Healthy people may suffer
only short-term symptoms such a high fever, severe headache, stiffness,
nausea, abdominal pain and diarrhea. In pregnant women, Listeria infection
can cause miscarriages and stillbirths. Contact SeaSpecialties at 305/621-7600,

* Eastside Deli Supply, Inc., located in Lansing, Michigan
has voluntarily recalled 'Eastside Deli', 'Fresh from the Deli' and 'In
Your Belly Deli' Products due to possible contamination with Listeria monocytogenes.
The recall includes all “Eastside Deli”, “Fresh from the Deli” and “In
Your Belly Deli” sandwich products up to and including products dated April
4, 2005. Numerous recalled sandwiches were distributed to convenience stores
in parts of Michigan, Ohio and Indiana. Most sandwiches have reportedly
been removed from circulation. For further information contact Eastside

education handout “Good Habits, Good Health” to avoid food or waterborne
illness:
* Avoid water that may not be safe. Boil drinking water
from 1-3 minutes. If you buy water, get spring water or purified water.
Water filters must meet the NSF Standard number 53 for "cyst removal".
* Wash fruits, vegetables and the tops of cans with safe
water before you open them.
* Keep utensils, food surfaces, and the sink clean. Use
hot, soapy, bleach water and rinse well.
* Wash your hands often.

* USDA advises putting "food safety" resolutions at the

announced its new Office of Food Safety, Defense, and Outreach (OFSDO).
The OFSDO consolidates the Center's outreach and education on food safety
and defense with its counter-terrorism and chief medical officer responsibilities.
The Office vision is "to increase CFSAN's recognition by the outside world
as a resource for information on food safety, food defense, nutrition,
and cosmetics." David Acheson, M.D., Chief Medical Officer, former Director
of the Food Safety and Security Staff (FSSS), is Director of OFSDO.
Center-wide outreach and education functions and associated
personnel have been merged into OFSDO, with delegated authority on Center-based
communication. The functions include consumer education and industry outreach
formerly within the Office of Constituent Operations (OCO), trade press
activities formerly within the Executive Operations Staff (EOS), labeling
and nutrition outreach formerly within the Office of Nutritional Products,
Labeling, and Dietary Supplements (ONPLDS), and outreach and education

Conference Information
The Nutritionists In AIDS Care (NIAC) and the HIV/AIDS
Dietetics Practice Group (DPG) of the American Dietetic Association (ADA)
had their 4th Annual NIAC & HIV/ AIDS DPG Conference: Leading-Edge
Update On Nutrition & Psycho-Social Management in HIV on March 5, 2005
at Hunter College, Brookdale Campus, in New York City, NY.
Conference objectives focused on:
* the importance of integrative and practical harm reduction
nutrition strategies as well as dispelling fad diets for people living
with HIV/AIDS;
* cultural competency of Caribbean & Southern cuisine's
as it relates to the management of optimal health, metabolic complications
in HIV such as lipodystrophy and HIV wasting syndrome;
* the relationship between mental health on proper nutrition
and the nutritional impact of illicit drug use;
* advanced nutritional counseling skills and the concept
of supervision for registered dietitians.

Spring 2005 HIV Nutrition Update
The Spring 2005 HIV Nutrition Update Newszine is now available.

There are two feature articles in this issue. The first
feature, "HIV Nutrition Crisis in New Delhi", offers insight on the HIV
pandemic in India where treatment for the disease is very minimal. Sandeep
Saluja, MD is the New Delhi physician who wrote the article based on her
practice of Internal Medicine for the past 21 years. Her article describes
vast differences in both attitudes and practices of physicians who see
people with HIV infection in India as compared to the U.S. The second feature
article describes preliminary research regarding dietary algae and HIV
disease progression. Article co-authors include Jane Teas, PhD who is an
Assistant Research Professor at the Arnold School of Public Health at the
University of South Carolina. After studying the health effects of algae
as a breast cancer preventive and reviewing published studies of its efficacy
against HIV, she began a small clinical trial of algae and HIV. Kelly N.
Patterson has more than ten years experience as an international development
specialist, former Reuters Health and Sciences journalist, and secondary
school teacher. Jill Royer, BS is a health educator who has worked in a
variety of medical and community sectors for the past 20 years. Jill continues
her work done in South Africa, including the ongoing “Indigenous Seaweed
Project”.

The AIDS Community Research
Initiative of America offers a variety of free resources for the HIV/AIDS
community. Among them is the acria UPDATE newsletter that has periodical
articles on HIV nutrition such as those in the Spring
2002, Volume 11, Number 2 ACRIA Update. ACRIA also offers a free
National HIV Treatment Education Technical Assistance Program for non-medical

Neuropsychiatric Complications and Vitamin B12
Recent reviews discuss the neuropsychiatric effects of
HIV infection and the antiretrovirals prescribed to treat HIV disease.
Review an audiovisual presentation
on this subject. Vitamin B12 deficiency is also associated with the use
of metformin, which is used to decrease blood glucose levels.
Vitamin B12 deficiency can result in neuropsychiatric
complications as well. Previous listserv messages
and HIV ReSource Review issues discuss nutritional
issues associated with B12 deficiency including neuropsychiatric complications.
Symptoms of vitamin B12 deficiency include abdominal discomfort, anorexia,
confusion, dementia, dyspnea, glossitis, loss of vibration sensation, mental
depression, neurologic disturbances, pallor, prolonged bleeding time, soreness
of mouth/tongue and weight loss. Vitamin B-12 status is also associated
with bone health.

* The 133rd American Public Health Association Meeting
and Exposition "Evidence Based Policy and Practice" will be held on November
5-9, 2005. See the Call
for Abstracts. Further conference information will be available through
the American Public Health Association web

* The Nutrition in HIV Care Conference Certificate Program
is designed for registered dietitians, dietetic technicians, dietetic interns
and other health care personnel. The Commission on Dietetic Registration
(CDR) categorizes the program as level one, which assumes that the participant
has little or no prior knowledge of the areas covered. The Conference is
sponsored by AIDS Project Los Angeles and was held on March 14, 2005 at
3530 Wilshire Blvd, 7th Floor, Suite 700 in Los Angeles, CA. For additional

Diabetes Updates
Numerous articles about metabolic syndrome resulting from
the use of highly-active antiretroviral therapy (HAART), including body
changes such as glucose abnormalities such as diabetes, are featured in
the HIV ReSource Review and HIV
Nutrition Update issues. The following recent resources are not specific
to HIV disease but they may still be useful to health care providers treating
people with diabetes and/or metabolic syndrome.
